The American Academy of Pediatrics recommends your child be seen at the following ages for routine healthcare:  birth/hospital discharge, 2 weeks, 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 24 months, 30 months, 3 years, and annually thereafter. Well-child checkups focus on health and developmental issues, and provide the opportunity to vaccinate your child against preventable childhood illnesses. Due to the time consuming nature of these visits, if your child is sick at the time or if numerous issues need to be addressed, a separate appointment will be necessary in order to adequately address all concerns.
Most school-sponsored athletics programs require a physical exam and physician clearance prior to participation. Please remember to bring your sports physical form with you to the appointment and have your Preparticipation History Form completed prior to your physical.

Forms (Camp/Sports/School): We are pleased to offer you a secure patient portal free of charge where you can register a new patient, update existing contact information, immediately access vital medical information (including growth charts and immunization records), view and pay balances and statements online, and much more.
